Who we serve:

The Psych Rehab Department serves the recovering mentally ill adults of Livingston County.  These consumers are age 18 and older and have a serious mental illness that has impaired their functional skills.  We provide transportation within Pontiac.

Our mission:

The mission of the Psych Rehab Department is to provide individualized mental health services to assist recovering mentally ill Livingston County adults to recover to the greatest extent possible, and to maximize their level of independent functioning and their integration in the community.

The services we offer:

  • Assessment and Treatment Planning:

    • Together we conduct an assessment and the consumer sets the goals for his/her treatment

  • Case Management:

    •   We provide support, advocacy, and assistance in accessing services within and outside the agency

    • Linkage with nursing homes, group homes, hospitals, etc.

  • Counseling:

    • We provide counseling for coping skills, financial management skills, etc.

  • Psychosocial Rehabilitation:

    • This is a consumer-centered approach to recovery involving:

      • Functional skills development

        • In group and individual formats

        • In agency, community, and home settings;

      • Peer support and socialization;

      • Consumer and community resource development

      • Vocational support services

  • Nursing Services:

    • Our registered nurses coordinate services with psychiatrists

    • Provide medication administration, training, and monitoring

    • Coordinate consumer services with local physicians, pharmacists, etc

  • Supported Residential Program:

    • Our nine-apt. complex houses eight consumers who live independently with minimal support

    • Includes one respite apartment used for transition from hospital to home, or for consumers needing increased assistance/monitoring

We encourage consumer involvement through

  • Participation in treatment planning meetings

  • Participation in monthly team meetings of consumers and staff

  • Monthly Crossroads Advisory group meetings

  • Open communication with case managers and with the department director
